Campus Ministry CSP Trip Sign-up Procedures:
- Permission slips for the CSP Trip will be available online by 4 p.m. exactly one week from the date of the scheduled trip. Permission slips are only available online.
- Permission slips that are altered in any way (either by hand or through a digital file) will be rejected. If a file is to have been found altered, the student’s name will be crossed off the list and will not be able to participate on that CSP Trip.
- Download the permission slip and make sure all information and parent signatures are complete on both pages of the form.
- Bring the permission slip to Campus Ministry (rm. 117) and place it into the appropriate CSP Trip folder. Sign your name on the list.
- Students may only sign up for the trip when they are able to place their signed forms into the folder.
- Permission slips cannot be faxed in.
- A student placing her name on this list without turning in a form first will be crossed off the list and will not be able to participate on that CSP Trip.
- Sign ups are on a first come, first serve basis. The first 8 girls who sign up with their filled out permission slips in the folder will be automatically placed on the trip.
- If a student comes and the list is full, she may still turn in her form and put her name on the waiting list.
- There have been times where a student who has signed up for a trip may have to back out. In that case, the first name on the waiting list will be bumped up.
